General Data Protection Regulation (GDPR)
If you are receiving treatment at Plexus Amsterdam, your physiotherapist keeps your patient record up to date. This contains all the personal and medical details that are relevant to the treatment. The recording of these details is subject to the GDPR, which sets out the following: 1. the purpose of the record, 2. which data is recorded, 3. which persons have access to the record, 4. the right of the patient or their legal representative to access the record.
Electronic patient record
- We work with an electronic patient record (EPR). Access to the EPR is subject to restrictions.
- Only data that is related to your treatment is entered in the record.
- You are entitled to access your patient record. If you think that your record contains incorrect data, you can request that this data is changed or deleted.
Confidentiality of patient data
- Besides the physiotherapist who is treating you, a number of other persons have access to your record, or to part of it. They are the other physiotherapists at the practice, any trainees and the administrative staff. All these persons have a duty of confidentiality.
- Your patient data is not disclosed to third parties or even to close family members. Your data is only disclosed following permission from you or from your legal representative. The record is not made public after your death either.
- Your patient record is not discussed in front of other patients or third parties.
- With regard to your treatment, it may be advisable to consult your doctor or specialist. If your treatment takes place following a referral from your doctor or specialist, then the patient’s permission is not required for this consultation. If your treatment takes place without a referral (direct access), then the physiotherapist must request permission for the consultation.
- If your patient data is important to scientific research or education, we can only use it after receiving your written permission. Preferably, your data is used in an anonymised form.
- On ending your treatment, your digital record will remain in the system until it is archived. From then on, your data is less accessible.
Treatment rooms
In order to protect your privacy, our treatment rooms are closed off. The physiotherapists have a mutual agreement that they do not enter the treatment room without permission.
If you think that your data has been dealt with in contravention of the GDPR, you can follow the complaints procedure of the Koninklijk Nederlands Genootschap voor Fysiotherapie (KNGF).
